Physical pain is most easily resolved with the administration of medications or a non-pharmaceutical intervention such as repositioning or the application of heat or cold. Nursing care in a general medical unit is about patient- and family-centered care, in line with Kolcaba's comfort theory. Most patients seen in this type of unit have multiple comorbidities and difficult social situations that require an assessment of their past health history, their support system and their current living situation. All of this is taken into account in multidisciplinary rounds where the data collected is communicated to all disciplines and a care plan is developed for each patient. The unit I currently lead brings together our multidisciplinary unit on a daily basis. Needs are identified and assigned to team members that include social work, care management and therapies as well as nurses and providers. Since the team meets daily there is the opportunity to evaluate the effectiveness of the prescribed interventions. Nursing care management is an integral part of this work as part of the assessment, planning and coordination of care in the hospital and when the patient returns home and into the community.
