Before the clinic started, I did my regular preparation for the next clinical days in week 3. I thought I was well prepared for the clinic, I knew the mental and medical conditions of the customer. But I was focusing more on the client's mental illness and not his medical illness, so when the nurse came to me and told me that Client W was short of breath, I was a little shocked because I didn't expect those unexpected events. Brief Introduction to the Clinical Situation The first day of the second clinical week, was the first day I had two clients in one shift. Client W and I were walking to the dining room and another client stopped me and asked me to put on my jacket, so I told my client I would meet him in the dining room. After I did that, I was walking to the dining room and Nurse A said that Client W was having trouble breathing and that we needed to give him his 9:00 am swabs. He had audible lung wheezing and his respiratory rate was rapid. So we had to give the client three different swabs, SalbutaMOL Sulfate is a bronchodilator that allows the lung alveoli to open so the next two swabs can be administered, ipratropium to help reverse airway obstruction due to COPD, and Advair diskus to his asthma. Client W was much better after receiving those inhalations, his breathing rate was stable and he was not wheezing like before receiving the infusion medications. We suggested that he should lie down for at least 30-60 minutes after administering the medication, just so he could get some rest for his lung. For some reason, he had gotten up and was walking around the unit and was feeling a little wheezing, but he turned around better as soon as we took him back to his room and stood by the bed with him, his breathing got much better with the position of Fowler in his bed. And then, after administering his 9am medications, he asked for Tylenol for his “severe” headache that many nurses had told me was probably just attention-seeking behavior. Because it would have been better as soon as I talked to him and took him back to his room and told him to lie down and have some deep breathing techniques.
