Introduction: This assignment involves researching and studying the facts surrounding the crash of Crossair Flight 3597 at Zurich Airport on 24 November 2001, capturing valuable lessons learned and expressing my views on the operational perspective, both safety and cost, during the approach phrase. The objective of this report is to discover and learn more about the facts and factors that contributed to the crash of Crossair Flight 3597. In addition to understanding how airline crashes can occur, knowing the importance of safety management and other factors, and what can I do to prevent such incidents from happening again. To begin this task, I will discuss the facts of the Crossair Flight 3597 accident, followed by three contributing factors involved in the context of the flight operations, and finally the conclusion and references.2. Crossair Flight 3597 Accident Facts:Crossair Flight 3597 will depart from Berlin Tegel Airport at 21:01 CET for Zurich Airport. It carries 28 passengers and 5 crew members, including Hans Ulrich Lutz, a 57-year-old captain, and Stefan Loehrer, a 25-year-old first officer. Approaching Zurich at 20:58:50 UTC, Crossair Flight 3597 received clearance from the air traffic controller to land on Runway 28 in poor visibility due to low clouds instead of Runway 14. Since Runway 28 is not equipped with ILS, pilots have to perform a VOR/DME approach where glideslope/localiser information is missing. The approach originally planned by the Commander on runway 14 was shelved due to the sudden change of approach runway. Therefore, the Commander requested the First Officer to seek another approach to Runway 28 by referring to the Jeppesen charts. A new set of parameters was found... halfway through the document... the acuity of the commander, his decision to violate the MDA, the lack of situational awareness in him and the lack of preventive action on the part of the first official. This was facilitated by Crossair's supervision of the captain's flying prowess and navigation skills. If all these errors had not been omitted, the chances of Flight 3597 crashing would have been minimized. However, through all the research and findings I have made, some factors that could lead to aviation accidents are unpredictable, all we can do is maintain high safety standards and fly to do our best to prevent such accidents from happening again.
